[인터뷰] 대한간학회 김윤준 이사장 “간암 정복 시대 열렸다…건보 급여 확대 '필수불가결'” ·간암 5년 생존율 40% 이상 상승, 근치적 치료 후 70% 달하는 높은 재발률 문제 ·재발률 40% 이상 낮추는 치료제, 건강보험 미적용으로 치료 제한 ·국가 차원의 간암 환자 치료 환경 개선과 건강보험 확대 위한 노력 필요 “불과 얼마 전까지만 해도 불치병으로 여겨졌던 간암이 이제는 완치를 바라볼 수 있는 시대가 됐다. 10%대에 불과했던 5년 생존율은 40% 이상으로 크게 상승했다. 그러나 근치적 치료 후 70%에 달하는 높은 재발률은 과제로 남아 있고, 재발률을 40% 이상 낮추는 치료제가 아직 건강보험에 적용되지 않아, 치료가 제한되는 문제도 있다.” 대한간학회 이사장이자 서울대학교병원 소화기내과 김윤준 교수는 10월 20일 ‘간의 날’을 맞아 진행한 인터뷰에서 국내 간암 치료의 현재와 미래에 대해 이같이 밝혔다. 간암 치료의 발전은 오랜 시간 동안 도전과 극복의 연속이었다. 25번째 '간의 날'을 맞이한 올해, 김 교수는 간암 치료의 과거와 현재를 돌아보며 그 변화의 크기가 실감된다며 소회를 밝혔다. 한때는 진단 후 몇 개월 만에 생명을 잃는 환자들이 많았지만, 이제는 생존율이 크게 증가해 환자들이 완치까지 꿈꿀 수 있게 됐다고 기뻐했다. 그럼에도 불구하고 여전히 많은 환자들이 늦게 진단받아 치료 기회를 놓치고 있는 실정이다. 특히 치료 후 재발률이 70%에 달해 간암 치료의 어려움을 더욱 가중시키고 있다. 여기에 건강보험 지원이 부족해 환자들이 선택할 수 있는 치료의 폭도 제한된 상황이며, 이는 간암과의 싸움을 더욱 힘겹게 만드는 요인이다. Q. 간암 정복을 위해 수십 년간 노력해 오셨다. 올해 25번째 '간의 날'을 맞이한 소감 부탁드린다. 30여년 전 처음 간암 환자를 진료하기 시작했을 때만 해도, 많은 환자가 늦게 진단받아 간 기능이 이미 많이 악화된 상태에서 발견되는 경우가 많았다. 간암 진단 후 3개월에서 6개월 이내에 사망하는 사례가 빈번했다. 간암이 발병하면 살아남기 어렵다는 인식이 생긴 것도 그 때문이다. 그러나 현재 한국은 전 세계적으로 간암 5년 생존율이 가장 높은 국가 중 하나가 됐다. 특히 5년 생존율이 40% 이상에 달하는 몇 안 되는 국가로, 이는 정말 놀라운 발전이다. Q. 간암 정복을 위해 세운 앞으로 계획은. 1차 예방과 환자 치료권 향상을 최우선 과제로 삼고 있다. 여전히 늦은 진단으로 치료와 수술이 필요한 환자들이 많다. 이들에 대한 적절한 치료를 위해 건강보험 급여 등재 기준이 완화되길 희망하며, 이를 위한 다양한 활동도 이어나갈 것이다. 최근 간암에 매우 효과적인 항암제들이 많이 개발됐다. 그러나 많은 항암제 중 현재 건강보험을 적용받는 약제는 티센트릭(성분명 Atezolizumab)과 아바스틴(성분명 Bevacizumab) 등 극소수에 불과하다. 환자의 치료 선택권을 확대해야 한다는 지적이 나오는 이유다. 무엇보다 간암은 재발을 막는 것이 중요하다. 치료 후 간암이 재발하면 사망으로 직결되기 때문이다. 간암 재발 방지 효과와 관련해 티센트릭과 아바스틴이 어느 정도 효과가 있다고 발표됐으나, 전 세계 의사들이 이에 동의하는 것은 아니다. 현재로서는 사이토카인 유도 살해세포(Cytokine-induced killer cell, CIK) 기반 면역세포 치료가 유일하게 간암 재발 방지에 효과적인 치료법으로 전 세계적으로 인정받고 있다. 즉, 이 치료에 대해 보험 규제를 보완해 더 많은 환자에게 치료 혜택이 돌아갈 수 있도록 하는 것도 최우선 목표다. Q. 최신 기술이 접목된 간암 치료제와 치료법이 다수 개발됐다. 여전히 미충족 의료 수요가 큰 이유는. 현재로서 완치를 기대할 방법은 간 이식, 간암 절제술, 혹은 고주파 열치료 정도뿐이다. 간암은 근치적 치료 후에도 재발률이 70%에 달할 만큼 매우 높은 재발률을 보이며, 결국 많은 환자가 사망에 이르게 된다. 이때 사이토카인 유도 살해세포(CIK) 치료가 재발 방지에 효과적이며, 부작용이 거의 없는 지씨셀(GC셀)의 '이뮨셀엘씨주(Immuncell-LC)'가 재발 방지와 치료에서 큰 역할을 하고 있다. 국내에서 이뮨셀엘씨주 치료를 가장 많이 시행하는 의사 중 한 명으로서, 이 치료법은 재발률을 40% 이상 낮출 수 있는 수술 후 재발 억제제로서 매우 효과적이고 비용 대비 우수하다고 판단된다. Q. 간암 치료에서 재발 방지를 핵심으로 꼽았다. 특히 이뮨셀엘씨주를 강조한 이유는. 재발 방지 치료에서는 부작용의 위험을 최소화해야 하며, 특히 치명적인 부작용은 절대 용납될 수 없다. 이러한 이유로 안전성과 치료 효과가 보장된 이뮨셀엘씨주를 간암 치료에서 강조하고 있다. 현재 모든 간암 환자에게 이뮨셀엘씨주 치료를 권장하고 있다. 이는 개인적인 판단이 아닌, 여러 주요 치료 가이드라인과 대한간암학회 가이드라인에서도 사이토카인 유도 살해세포(CIK) 치료가 명시돼 있기 때문이다. 특히 관련 데이터가 매우 확고하다. 실사용데이터(Real World Data)뿐만 아니라, 통제된 무작위배정된 임상 데이터 등으로 발표된 논문에서도 이뮨셀엘씨주의 안전성과 유효성이 입증됐다. 이러한 선택지를 환자에게 널리 알리는 것이 의사로서 본분을 다하는 것이라고 생각한다. 다만, 이뮨셀엘씨주가 아직 건강보험 적용을 받지 못해 높은 치료 비용이 가장 큰 걸림돌이다. 앞서 언급했듯이, 간암 환자들이 어려움을 겪는 이유는 간암이 재발을 반복하면서 사망에 이르게 되는 경우가 많아서다. 이뮨셀엘씨주가 간암 재발률을 40% 이상 낮추는 만큼, 모든 환자에게 반드시 이뮨셀엘씨주를 소개해 선택의 기회를 제공하고 있다. ~링크 기사 전문 https://lnkd.in/g4ssMyAK #간암 #항암제 #면역세포치료제 #지씨셀 #이뮨셀엘씨주

We're excited to announce that GC Cell has received #MFDS clearance for the #IND of our Phase I trial of #GCC2005, an allogenic anti-CD5 CAR-NK therapy targeting T-cell malignancies. #GCC2005 aims to overcome key challenges of current CAR-T therapies, such as fratricide and T-Cell Aplasia, while offering improved durability and efficacy. This could provide broader and more affordable treatment options for various lymphoma subtypes.
