This month we are celebrating 10 years of #MAGICapp, as the first version of MAGICapp was launched In November 2013 🎉 In this piece, we share some reflections on where MAGICapp started and where it is heading to. 💥 Workshop announcement: MAGICapp (www.magicapp.org) provides a solution by offering a web-based authoring and publication platform designed for evidence users, systematic reviewers, guideline developers, and proponents of shared decision-making. The platform enables efficient authoring and updating of evidence, recommendations, and decision aids, and extensive opportunities for dissemination, adaptation, and implementation. The objectives of this workshop are: Demonstrate and discuss the value of creating guidelines in a digital guideline platform for improved authoring efficiency and increased dissemination opportunities. Learn the basics of using MAGICapp with hands-on practical work including how to author and update guidelines. 💥 Hot off the press: Recommendations on proactive therapeutic drug monitoring (TDM) of biologic drugs in adult patients with inflammatory bowel disease #IBD, inflammatory arthritis, and psoriasis produced with the support of MAGIC and housed on magicapp.org and BMJ, have been published Content on BMJ: https://lnkd.in/dycHDum2 Content on MAGICapp: https://lnkd.in/dZhGzsRn Proactive TDM refers to the measurement of drug concentrations and anti-drug Abs at timed intervals irrespective of disease control. Proactive TDM aims at optimising individual pt dosage regimens (improving outcomes) The scope of this guideline was: P: Adult patients with inflammatory bowel disease, inflammatory arthritis or psoriasis at the start or maintenance therapy with biologic drugs I: Proactive TDM C: standard or care (no TDM or reactive TDM) O: metrics of disease control/remission and safety T: the longer the better (48-64 weeks) Summary of guidance: 1. Weak recommendation in favor of proactive TDM for intravenous infliximab during maintenance therapy for adult patients with inflammatory bowel disease #IBD, inflammatory arthritis, or psoriasis 2. Weak recommendation against proactive TDM for adalimumab and other biologic drugs during maintenance therapy for adult patients with inflammatory bowel disease #IBD, inflammatory arthritis, or psoriasis 3. Weak recommendation against proactive TDM for intravenous infliximab, adalimumab, and other biologic drugs during induction (start) of treatment. This guideline assumes the individual perspective. It followed the standards for developing transparent and trustworthy guidelines and used the GRADE approach to assess the balance between benefits, harms, and burdens of the intervention. An international panel composed of patient partners, content experts, methodologists, and pharmacologists produced this guideline based on a linked systematic review and metanalysis (SR/MA) led by #DenaZeraatkar and team that identified 10 trials (2,383 pts) The SR/MA has been simultaneously published on the BMJ Med: https://lnkd.in/dgpqGScW When considering proactive TDM, clinicians and patients should engage in shared decision-making to ensure patients make choices that reflect their values and preferences. The availability of laboratory assays to implement proactive TDM should also be considered. Further research is warranted and may alter recommendations in the future. I had the opportunity to spend a week in Lilongwe, Malawi 🇲🇼 and conduct user testing interviews with health care personell for the guideline summary on enteral feeding for children, which we developed in the past months in connection to the #GELA project. GELA (Global Evidence, Local Adaptation) is an #EDCTP project with the aim of reducing child mortality in sub-Saharan Africa by developing better #guidelines for managing newborn and child health. We learned a lot from the local health care professionals and will use the insights to improve the next version of the infographic. Once again, it became very clear that local context and cultural perceptions are crucial when it comes to dissemination of health information.
