Aide Health is now supporting people in London 🙏 NeoHealth Primary Care Network, a network of six GP practices representing around 30,000 patients, has commissioned Aide to support its population living with long-term conditions. The PCN has previously been honoured with the ‘Best PCN Newcomer of the Year’ award in recognition of its innovation and commitment to improving patient outcomes. Dr Yasmin Razak, GP Partner at Golborne Medical, said: "We are continuously looking to develop new ways of working with people who frequently visit their GPs and improve our ability to prevent unnecessary emergency hospital admissions. Looking at new solutions for long-term condition management is an important part of that work. We’re excited to give our patients the opportunity to use Aide Health while we continue to encourage active participation in all aspects of their health.” The PCN will start by offering Aide to people living with type 2 diabetes and hypertension. 💙 Aide can now support people living with COPD. COPD affects approximately 2-3% of the UK population. Despite its lower prevalence, COPD outpaces asthma by around 4x in healthcare utilisation and accounts for 1 in 8 emergency hospitalisations. Alongside existing pathways for asthma, type 2 diabetes, and hypertension, Aide helps people with COPD self-manage by: - Improving medicine adherence - Quickly uncovering challenges they may have with their medicines - Monitoring their health (including the COPD Assessment Test) - Providing bite-sized educational content and signposting This new pathway is a continuation of our work to support respiratory conditions that began in 2022. People using Aide to manage their asthma have shown increased patient confidence (+8.4% in medicine understanding and ability to take them as prescribed) and adherence (raised to avg. 73.3%) while helping clinicians stratify risk at the population level. Patients trust AI for its lack of judgement and fear its 'coldness' more than the potential for human error. This contradiction, highlighted by two studies in this article, defines the challenge of designing for patient trust. Patient-facing AI, prescribed or otherwise, is a bell that cannot be unrung. Health outcomes are already somewhat dependent on not only how good but also how trusted these systems are. In this article, my thoughts on: - The intentional design of neutrality - Why cultural competence is critical for AI in healthcare - Using shared decision-making to minimise detachment Additional input is much welcomed. Well Up North Primary Care Network chooses digital long-term condition platform Aide Health to support its patients. With a population of 61,000 patients, Well Up North PCN comprises mostly rural, seaside, and small market town communities. The unique nature of this geography means the PCN covers more square miles than Greater London, with many patients living 60 or more miles from the nearest A&E. Dr Neil Forster, GP Partner at Union Brae Surgery, said: "Our patients can be met with rural isolation and limited access to public transport. If patients can’t come to us, it’s vital we go to them to effectively manage the caseload of long-term conditions. Crucial perspective from Paul Corrigan 💡 Improving a patient's capability to self-manage is associated with reducing utilisation in both primary and secondary care. Dr Steve Laitner, Aide Health Head of Population Health, said, "As we move from hospital care to primary and community care, we must also move from primary and community care to self-care. This is not only an economic imperative but also a moral one, as we know that informed, empowered or “activated” patients have much better outcomes. Self-care needs to be supported so patients are not left on their own. Support can come from friends and family, other patients, NHS professionals and, importantly, technology. The NHS doesn’t have to do it all, and it shouldn’t. Only by supporting people to care for themselves can we ensure the NHS is there for us when we really need it."
