    These new graphs may be the most important in the NHS and wider public sector right now. They show that Sussex completely bucked the trend for rapidly rising waiting times for physiotherapy appointments. While they went up across the country (the bottom graph), they actually fell and then stabilised in Sussex (the top graph). This was done not by spending millions on AI or processing patients more quickly through medical consultations. It was achieved by moving to more human, holistic care that drew on the power of the community. It’s an approach that has applications well beyond musculo-skeletal conditions and thus offers a hugely valuable insight into the shifts that are needed to end the crisis in the NHS and wider public services. At the heart of the approach are Community Appointment Days (CAD) which I have written about elsewhere. These are mass events where hundreds of patients have human, non-medicalised conversations with clinicians about what really matters to them and are immediately introduced, where appropriate, to community support. The graphs come from the just published independent evaluation of the CADs. But speak to those who have designed and delivered the CADs and they will tell you three very important things: 1) The success in Sussex is down to a wider culture change in mindset and practices in the whole team, not the CAD initiative alone. I would summarise that as a shift from a ‘do to’ to a ‘do with’ approach that sees the public sector institution working with individuals, their networks and wider community as equal partners. 2) The overarching aim is to provide more holistic, human care NOT reduce waiting times - that is merely a side effect. Indeed, sticking with the productivity, process-oriented mindset which has infected so much thinking and practice across the public sector is totally counter-productive. *The secret to radically shorter waiting lists is better care not quicker care!* The “overwhelmingly positive experience” reported by patients proves that point particularly at a time of declining public satisfaction with the NHS. 3) As such, the key to replicating Sussex’s success is not to ‘lift and shift’ the CAD model but to understand the ‘do with’ principles behind it and apply them in the very different contexts that exist across the public sector, co-designing and co-delivering a new approach with people, community and staff.
