This week is #MedSafetyWeek, focused on the theme "the importance of using medicines in the right way to prevent side effects, and to report side effects when they do occur".  In the UK, the Yellow Card Scheme helps the Medicines and Healthcare products Regulatory Agency (MHRA) monitor the safety of all healthcare products to ensure they are acceptably safe for patients and those who use them. New on the hub today, Tom Bell shares his experience of being a Patient Safety Partner for an NHS Trust. Having been invited to the role by the Trust, Tom describes his disappointment at the Trust’s failure to value his experience and time. He describes the Trust’s approach to working with Patient Safety Partners and implementing PSIRF as tokenistic and disjointed. 🔧Good practice toolkit🔧 Intravenous therapy is an essential aspect of modern healthcare. While the benefits of using intravenous therapy usually outweigh the risks, occasionally the administration of IV therapies can go wrong. Infiltration and extravasation is a complication whereby the drug or IV therapy leaks into the tissues surrounding the vascular access device. Featured on the hub, a toolkit intended to enable local services and healthcare organisations to implement polices, protocols and guidelines that will increase awareness about non-chemotherapy extravasations. This toolkit, developed by the National Infusion and Vascular Access Society (NIVAS), is shaped around a five-stage approach: 1️⃣ Prevention – Safe IV therapy administration and vascular access practice. 2️⃣ Recognition – Recognise the early stages of extravasation. 3️⃣ Treatment – Early intervention and treatment to reduce or stop tissue damage. 4️⃣ Follow up – Ensure the patient is followed up and supported. 5️⃣ Reporting – Standardised incident reporting of infiltration and extravasation.
