Our Vision is: A health system that reduces health inequalities; where people are more active participants in their own health and wellbeing, and the strengths of all sectors – particularly the voluntary sector - are maximised. Our Mission is: To develop and test innovative programmes and prototypes that improve health and wellbeing, tackle health inequalities, and reduce demand on NHS services. We explore and test new ways of working together and delivering services to patients, showing real-world impact. Our approach: 1. We believe there is strength in collaboration 2. There is an urgent need to shift our health system towards prevention, and to support people with long-term conditions earlier and more effectively. Around 25 million people in England live with a long-term condition and about a quarter of the population have two or more conditions. Social prescribing offers opportunities in both Primary Care and Secondary Care to support people facing social issues such as loneliness, debt or housing issues. There are a wide range of projects that demonstrate the crucial role social prescribing can play in the prevention and management of conditions like diabetes, cardiovascular problems, dementia and cancer.
