Nevertheless, regardless of the technique used, there are some common steps that should be taken to ensure a successful RCA. To begin, you must define the problem by answering questions such as what happened, when, where, how, and who was involved or affected. Then you need to gather data which could include evidence, logs, reports, feedback, or other information to support your analysis. Afterward you have to analyze the data to identify the possible causes, contributing factors, and consequences of the problem and how they relate to each other. Subsequently you must identify the root causes and verify or validate them. Once this is done you can recommend solutions and implement them by executing, monitoring, and evaluating them while communicating and documenting your results and lessons learned.

Effectively utilizing the 5 why analysis can help narrow down what happened. The Problem manager can act as a moderator and maintain the culture of focussing on what and why, rather than who. Instead of finger-pointing or assigning blame, we should think together about how we can collaborate and ensure that Major Incidents never happen again. We can also focus on improving processes, and adequate training of resources if need be.
