A4H provides advocacy, research and communications to develop models that improve care, reduce fragmentation in care delivery and lower costs. For one family doctor, it’s collaborating with social workers and case managers, made available through an Innovation Center pilot program, who work with her to address the unique needs of patients and reduce avoidable hospitalizations. A patient with chronic obstructive pulmonary disease told her that because of this extra support he no longer feels like he must go to the emergency room every time he’s short of breath. A strength of accountable care is the ability to adapt to local challenges. A recent blog from Aledade, Inc. CEO Farzad Mostashari, a member of the A4H Executive Council, documents a recent road trip visiting a wide range of practices succeeding in accountable care, including rural practices and solo practitioners. Each of these unique site visits demonstrate how accountable care expands high-quality care in local communities while adapting to the needs of their patients. Last month, Sean, Joshua and I drove cross-country and stopped every day at an Aledade practice and visited with our local Aledade teams. My only regret is that we were only able to touch a tiny fraction of Aledade Nation. Here are some of my reflections.

Based on data from 2022, nearly half of traditional Medicare beneficiaries nationwide are participating in accountable care, improving their health outcomes and experiences, and lowering costs. Several states have accountable care participation over 50%. In July, Accountable for Health submitted comments on CMS’s proposal to mitigate the impacts of SAHS billing activity on the financial performance of MSSP ACOs. Because of their focus on data and population health information, ACOs are uniquely well-situated to identify early aberrant billing patterns, unusual utilization, and spending trends. We are pleased to see CMS taking steps through rulemaking and the recently released Medicare PFS Proposed Rule to create pathways for recognizing capability and for ensuring ACOs are held harmless for current and future SAHS billing activity. Last week, members of the Accountable for Health (A4H) Executive Council, including Chris Riopelle, CEO of Strive Health, Srin Vishwanath, Founder and CEO of OPN Healthcare, Scott Lloyd, Chief Development and Strategy Officer of Evergreen, and Sean Cavanaugh, Chief Policy Officer of Aledade, joined A4H CEO Mara McDermott in DC for meetings with Members of Congress, Committee staff, and Administration officials. A4H members emphasized the importance of continuity strategies for CMS Innovation Center models, extending and then restructuring the alternative payment model (APM) bonus, and how accountable care expands access to high-quality care. Thank you, Senator Marsha Blackburn (R-TN), Congressman Darin Lahood (R-IL), Saha Khaterzai, Abigal Chance, Santiago Gonzalez, Melanie Egorin, and Stacy Sanders for taking the time to meet with members of A4H and discuss some of the most pressing issues facing accountable care.
