CMMI's announcement last week to end four models early represents a strategic pivot rather than a retreat from value-based care. Three key takeaways: 1️⃣ The administration continues to emphasize total-cost-of-care risk models. While CMMI is shuttering the Maryland model, this is more plausibly explained as being due to its limited scope. Indeed, CMMI stated that ending Making Care Primary and Primary Care First "does not change CMMI's interest in supporting primary care" but reflects "an effort to focus on different approaches that produce savings." This signals a commitment to effective risk models, not abandonment. 2️⃣ We're witnessing intentional model consolidation. By trimming these programs while projecting $750M in operational savings, CMMI is responding to congressional scrutiny (Sen. Blackburn recently suggested CMMI should "get to work or be shuttered") with a more focused portfolio. Secretary Oz's defense of CMMI suggests the administration is backing a leaner, more efficient approach. 3️⃣ ACO REACH's notable absence from the cut list speaks volumes. As CMMI approaches decision points on REACH's future beyond 2026, its preservation amid these cuts (in addition to its similarities to the first Trump administration’s GPDC model) indicates the administration likely values the program's potential and focus on full-risk. Rather than elimination, we might anticipate extension with refinements in 2027 and beyond. What's your take?
