    An organization can cause safety failures. When a safety failure happens in your organization, it's crucial to dig deep. Too often, investigations halt after scrutinizing the conduct of the individuals involved. But that's not enough. What systemic issues might have contributed?   Take a step back and examine both individual and organizational factors.     OSHA Updates to the Animal Slaughtering and Processing industry.   A new OSHA memorandum provides updated guidance for inspections in all animal slaughtering and processing establishments (NAICS 3116), expanding beyond previous rules that focused only on poultry processing. OSHA broadened its scope due to the industry's high injury and illness rates, aiming to reduce these risks through enforcement, compliance assistance, and outreach.   Under this new memorandum, all programmed and unprogrammed inspections in NAICS 3116 shall cover the focus hazards associated with the list below, in accordance with the FOM: Sanitation and Cleanup Operations Ergonomics/Musculoskeletal Disorders Personal Protection Equipment (PPE)/Payment for PPE Hazardous Energy (Lockout/Tagout) Machine Guarding Slips, Trips, and Falls Process Safety Management - Ammonia Chemical hazards -Hazards -Ammonia, Chlorine, Hydrogen Peroxide, Peracetic Acid, Carbon Dioxide, Nitrogen (gas & liquid) Occupational Noise Egress and Blocked Exits Recordkeeping   For more information, please visit: https://lnkd.in/gf87VGAn.   Reach out to your Acrisure risk advisor for more information and to discuss how this might affect your organization.     I had the honor to present Roadmap to Fleet Safety to Network of Employers for Traffic Safety (NETS). I learned a tremendous amount at this event. NETS has access to significant amounts of data that drives results. My presentation covered 5 stops on the map: 1. Organizations need to change how they think about safety. You have to change beliefs before you change behaviors. 2. Robust safety performance starts with leadership. It does not happen without deliberate effort. Effective leaders recognize that safety is at the foundation of operational excellence. 3. Safety has to be employee owned. This requires an emotional appeal and a commitment to a just culture. 4. Safety requires unconditional respect for process. Process must be understandable and employees have to be trained to SOPs. Consider estabilishing life critical rules, which are process violations that have potential for more serious outcomes. 5. Safety requires both individual and organizational accountability. A company can cause a crash as easily as an employee. Be sure all incident investigations look at individual and systemic contributors.     Do you know what types of incidents must be reported to OSHA and the specific timeframes for each? It's crucial for all employers to be aware of these requirements to ensure compliance and maintain a safe working environment. Knowing what incidents need to be reported and when can help you respond promptly and effectively in the event of a workplace accident or injury. Reportable Events: • Fatalities: Must be reported within 8 hours (if occurring within 30 days of the incident). • Hospitalizations, Amputations, Eye Loss: Must be reported within 24 hours of learning about it. All employers under OSHA jurisdiction must report these incidents, even those exempt from routine record-keeping.
