📣 Public Feedback Opportunity on Healthy People 2030 📣 The U.S. Department of Health and Human Services (HHS) is seeking public input on proposed new objectives for Healthy People 2030, a national initiative aimed at improving the health and well-being of all people over the course of a decade. 🏥 What is Healthy People 2030? Healthy People provides a comprehensive framework of measurable goals and evidence-based resources for addressing the most pressing public health challenges. This initiative includes objectives in areas such as health equity, chronic disease prevention, maternal and child health, mental health, and social determinants of health. Healthy People guides national health policy and resource allocation, providing benchmarks for federal agencies and public health professionals to monitor progress and assess the health status of the U.S. population over time. Here are just a few of the newly proposed objectives: 🏡 Environmental Factors in Health: Emphasis on integrating environmental health considerations into clinician training programs to address how environmental exposures impact patient outcomes 💉 Preventive Care for Pregnant Women: Focus on ensuring pregnant women are receiving preventive care (in this case, their Tdap vaccines) during the prenatal period 🫀 Reducing Hypertension Rates in Pregnancy: Targeted efforts to reduce pre-existing and pregnancy-associated hypertension among pregnant women, aiming to improve health outcomes for both mothers and babies 🔗 Learn more and share your thoughts on these important objectives: https://lnkd.in/e4y57KCu #PublicHealth #HealthyPeople2030 #HealthPolicy #HealthEquity #MaternalHealth #Hypertension #EnvironmentalHealth #PreventiveCare #SDOH

About us
Aligning for Health is made up of stakeholders who recognize that health is connected to more than just access to medical services, and that government programs promoting well-being, especially among low-income Americans, may be able to more effectively address overall health with more integration. There is a significant body of academic work showing that economic and social conditions have a powerful impact on individual and population health outcomes, as well as medical costs. These non-clinical factors – such as housing, food assistance, income, employment status, education and transportation – have the potential to contribute to health outcomes more than clinical health care. In fact, one widely cited study found that while ten percent of health outcomes in the U.S. are due to clinical health care, social and environmental factors are estimated to account for 60 percent of health outcomes. Through a combination of research, policy development advocacy, coalition building, and thought leadership, Aligning for Health seeks to energize stakeholders and policymakers around the need for integrated, cross-system strategies and sustainable financing models that achieve better health outcomes for beneficiaries, improve program effectiveness, and lower health care costs. Members include American Hospital Association, Blue Cross Blue Shield Association, CareSource, findhelp, Healthy Alliance, Signify Health, Socially Determined, UPMC, Unite Us, Vizient, and 3M.

Such a great session and range of speakers at the “Prescription for Change: The Intersection of Aging, Health Equity, and Federal Policy” at #CBCFALC53! The panel provided a wonderful range of perspectives on health equity and inclusion and the road ahead for addressing systemic disparities. Powerful Moments: ⚡ Rep. Clyburn opened with the story of his late wife Emily and her experience in navigating the healthcare system as a medical librarian with chronic conditions. His call to action for the audience was to keep pushing to make healthcare accessible and affordable. ⚡ Roslyn Young-Daniels led the panel through important questions around the urgency of health equity for seniors and opportunities for federal policy changes. ⚡ Dr. Jennifer Lewis shared the importance of diversity in clinical workforce to build trust with patients and meet their health needs. ⚡ Carl V. Hill underscored the foundation of health equity is to take a different perspective to understand individuals and their needs and that participatory engagement is critical to building awareness of conditions and treatments by fostering community trust. ⚡ Sue Peschin reflected on the need for inclusion of older adults in clinical trials and the role of clinicians in sharing information about clinical trials and how to get involved. ⚡ Donna Cryer emphasized the impact of community navigation in ensuring that older adults can get help in accessing care and gain an awareness of cures, medications, and treatments from individuals who look like them and live where they live. #CBCFALC #HealthEquity #CommunityNavigation #Disparities #Aging

Food insecurity remains a significant challenge in the U.S as outlined in the USDA’s "Household Food Security in the United States in 2023" report. 5 Key Takeaways from the Report: 🥑 Increased Food Insecurity: In 2023, 13.5% of U.S. households experienced food insecurity, a rise from 12.8% in 2022. 🥗 Very Low Food Security: 5.1% of households faced very low food security, where normal eating patterns were severely disrupted. 🧒 Children Affected: Food insecurity affected 8.9% of households with children, with 1% experiencing very low food security among children. 🏢 Federal Assistance: 58% of food-insecure households participated in federal nutrition programs like the Supplemental Nutrition Assistance Program (SNAP). 💰 Food Spending Gap: Food-secure households spent 16% more on food compared to food-insecure households. Federal programs like SNAP continue to be vital to America's safety net and to reducing the number of children, adults, and households that experience food insecurity. There's much work we need to do to drive system's changes that address the social drivers of health. 📑 Read the USDA's report here: https://lnkd.in/eunfSDq9 💻 View our webinar on food access here: https://lnkd.in/ePgY47Kv #FoodInsecurity #USDA #FoodSecurity #SNAP #HealthEquity

Aligning for Health member Findhelp is doing great things in New York! Learn more about how they’re working with Social Care Networks across the state. #socialdeterminants #healthequity #Medicaid
Congratulations to New York’s Social Care Networks (SCNs)! 🗽 Under New York's section 1115 Medicaid waiver, the SCNs will play a pivotal role in fundamentally changing the State's social care delivery system. We're honored to partner with several SCNs via our technology that enables eligibility determination, health-related social needs screening, closed-loop referrals, and social care payment and claims submission. We look forward to working closely with the New York eHealth Collaborative's Statewide Health Information Network of New York (SHIN-NY) and the Qualified Entities to support interoperability and standardized social care data exchange. The SCNs implementing the Findhelp platform will help the New York State Department of Health maximize its waiver investment, and will promote effective use of these state and federal dollars. Stay tuned in the coming weeks for more information about these exciting SCN partnerships!
