About us

Transforming the Culture of Care! Providing compassionate care begins with a secure, warm setting. At Azura Assisted Living and Memory Care we know that when our residents feel at home, they respond more positively and interact more with others. Our relationship-based MOSAIC care philosophy recognizes each person as an extraordinary individual and focuses on engagement. Azura Assisted Living and Memory Care and our parent company, Azura Living, have earned a reputation for being experts and elevating the standard of care with creative services, innovative therapies, and caring relationships. In fact, we have created our own evidence-based, cutting-edge training called MOSAIC to ensure that every team member has an enhanced understanding of our residents and those with memory loss, while recognizing how to personally communicate with and engage those within our family of care. We are blessed to have numerous assisted living and memory care communities in Wisconsin with continued expansions in the works! Please take the time to learn more about the community closest to you: Beloit, Brookfield, Clinton, Eau Claire, Fox Point, Kenosha, Manitowoc, Monroe, Oak Creek, Oconomowoc, Oshkosh, Sheboygan, Stoughton and Wausau!

  • Core Value Winner Wednesday 💙 We are so excited to announce this quarter's Core Value Winners! Our team members have gone above and beyond, embodying the values that are at the heart of our company culture. Join us in congratulating... Paige Pickard, Quality of Life Coordinator at Azura of Manitowoc Paige’s nominator shares, “The beautician came in to cut all of the resident’s hair today. When the beautician got here, she let me know that one of our resident’s guardians did not have anything set up with her to provide hair care to this resident. She was badly in need of a trim, but did not have the funds to do so. On her break, Paige went to an ATM and pulled $20 of her own money so this resident could have her hair cut. The resident was so thankful, and had the biggest smile on her face after the cut. Thank you, Paige, for being so incredibly kind to this resident and making her day!”

  • Core Value Winner Wednesday 💙 We are so excited to announce this quarter's Core Value Winners! Our team members have gone above and beyond, embodying the values that are at the heart of our company culture. Join us in congratulating... Morgan Cich, Caregiver at Azura of Monroe Morgan’s nominator shares, “We have a resident who is struggling to do her hygiene cares. She usually declines assistance because she is fiercely independent and very modest. Morgan convinced this woman to let her help in the shower by telling her it was a special day and offered a treat afterwards. Morgan took extra care to protect the woman’s modesty and dignity by covering the parts of her body that were not being washed at that moment. In this way, Morgan developed a trusting relationship with the resident and will likely be more accepting of help in the future.”

  • Thank you, thank you, thank you Mukwonago! Thanks to the overwhelming support of our community, Azura of Mukwonago's groundbreaking ceremony was a huge success! Your presence means the world to us as we take this important step in creating a supportive, loving environment for our future residents. Together, we’re building a place where dignity, care, and compassion come first. A special thank you to our shovel holders that took part in our ceremony: Fred Winchowky, Mukwonago Village President Stephanie Ziemke, VP of Development for the Alzheimer’s Association Dale Porter, President of the Mukwonago Area School District Board Alison Gorham, Vice President and Director of Wisconsin Operations for McShane Construction Company Dan Schindhelm, Project Manager for EUA Matt Lyons, Director of Development at Azura And, a thank you to everyone who donated raffle baskets, as well as our Azura team. • A #MOSAIC Dream Come True for Lieu 🎓 What would your best day ever be? Would it be a day on the golf greens, meeting your favorite celebrity or sitting poolside? Azura Memory Care and Assisted Living asks every resident this very question, and connects with local businesses and people to make their dreams come true - this program is known as the MOSAIC Dreams program. In the years that the MOSAIC Dreams program has been around, Azura has had a resident dance at the halftime show with the Milwaukee Bucks Grand Dancers, fly in an airplane and a helicopter, golf with a world champ, and more! Lieu, a resident at Azura of Stoughton, had the honor of earning her GED thanks to Stoughton High School and the MOSAIC dreams program on Wednesday, September 25th. Lieu was born in Saigon, and moved to the United States in 1972. She is the proud wife of an Army veteran, and lived in both Germany and Thailand during his time in the service. When she moved to the United States, she had started on the path to earn her GED through Blackhawk, but her admission into Azura prevented her from ever finishing the program. When team members at Stoughton High School learned of Lieu’s story, they were eager to give her the opportunity she had worked so hard for, but dementia had prevented her from achieving. Lieu was presented with an honorary GED from Stoughton High School, and her neighbors at Azura threw her the best graduation party! We are so proud of Lieu for her determination and hard work! Congratulations, graduate!

  • Core Value Winner Wednesday 💙 We are so excited to announce this quarter's Core Value Winners! Our team members have gone above and beyond, embodying the values that are at the heart of our company culture. Join us in congratulating... Jayden Hodgson, Homemaker at Azura of Eau Claire Jayden’s nominator shares, “While assessing a resident, I noticed they had been incontinent and would need a shower and bed change. I saw a few team members down the hall and asked if they would help me - Jayden immediately said yes and sprang into action, helping me shower this resident who had many difficult expressions. Jayden calmly talked the resident through the shower, cracking jokes with her and making her feel comfortable. Jayden dropped what she was doing to assist me through the shower and ultimately made the experience for the resident better than I could have hoped. Thank you Jayden!!”
