How confident are you that your patients will not develop urosepsis? In this insightful research letter, Biebelberg et al from Massachusetts General Hospital Dept. of Emergency Medicine highlight conflicts between the recommendations of the Infectious Disease Society of America and the Society of Critical Care Medicine on whether to initiate antimicrobial therapy for suspected UTI. Providers perform a high-wire act of prioritizing antibiotic stewardship vs the deadly consequences of urosepsis. They need more accurate and actionable diagnostics to support better outcomes. At Bacterioscan, it's our mission to empower providers with timely and trusted testing for UTI.

This is a fantastic review giving the history of how we got where we are in the diagnosis and treatment of UTI, from one of the leading minds in the field. This nugget really hits home for us at Bacterioscan: "Despite the high incidence of UTI diagnoses, the high cost of UTI treatment, and the increasing concerns associated with microbial resistance, the development of novel and more accurate UTI tests has not been considered a priority, largely due to other compelling public health concerns (e.g., the COVID-19 pandemic), but also due to the general perception that current UTI care is “adequate”" If you are a lab director or other provider who wants to do better, faster UTI testing for your patients, let's talk!

A 2020 analysis of Medicare claims data found that patients in emergency departments with dementia were more than twice as likely to be diagnosed with a UTI than patients without dementia, despite a lower prevalence of UTI-specific symptoms or complaints. The authors question "whether the higher proportion of UTI diagnoses in people living with dementia reflects a different presentation of true UTI disease versus a more liberal approach to UTI diagnosis in patients who are less able to articulate their chief complaint." A test system like the 216Dx that provides accurate results on an actionable timescale has the potential to reduce overdiagnosis of UTI and the incorrect treatments that may be prescribed as a result.

Many past studies have shown that urinalysis (UA) lack sensitivity for bacteriuria. But, UTI requires both bacteriuria and specific signs/symptoms of infection. This new study from Advani et al shows that urinalysis also falls short of being sensitive enough to rule out symptomatic UTI, especially in older female patients. All the while, overly broad reflex parameters 'rule-in' far too many samples for culture, overburdening labs and leading to excessive antibiotic use and associated harms like C. diff infection. The Bacterioscan 216Dx offers superior sensitivity for UTI with same-day results while also dramatically reducing the need for urine cultures. #answersbeforeantibiotics
