Make sure to check your kids' Halloween candy for flawed research based on bad data! 80% of Physicians are not employed by health systems or corporate entities despite the dramatic headlines that Physicians Advocacy Institute (PAI) and Avalere have been handing out all year. When this research was initially published, some people reached out with skepticism of the report as HDA recently published an analysis showing 66% were part of Health Systems and Large Groups. We conducted extensive QA to confirm that our numbers were correct, so we were also quite skeptical (We randomly sampled providers out of each “bucket” and had a researcher confirm the employment and group size) We were curious what would cause such an overestimation, so we dug into the actual report. There we found that it was based on the IQVIA OneKey database which only included affiliation information on 648,341 physicians (65% of the widely accepted physician count) We also discovered the methodology is primarily based on either physician address or pharma sales rep information. This significantly over classifies independent physicians as hospital employed when they rent offices adjacent to the hospital. Even the AMA was skeptical after their report 2 years ago and suspected they were classifying practices as corporate owned based on the legal entity type. In new Avalere research, also based on IQVIA data, they state that the Permanente Medical Group is corporate owned (as opposed to health system owned). We won’t even dig into the strange large swings we found in the year over year numbers. (Indicating the changes are driven by bias in the dataset, not reality) While the difference between 23% and 34% doesn’t seem very significant, it actually means there are 48% more independent physicians than are being reported here! We’re not saying physician consolidation isn’t a problem, but better data and research is required for everyone to truly understand what is going on.

Check out our new overview of physician-owned ambulatory surgery centers (ASCs) on our blog!
Ambulatory Surgery Centers (ASCs) are a growing cost-effective alternative to hospitals for an increasing number of medical procedures. Cardiology is a notable addition as of late. These procedures are generally done cheaper while maintaining or improving quality. This shift benefits payers and patients through lower costs. Physicians can also make more by capturing part of the facility fee through ASC ownership. Hospitals, however, are challenged as they receive higher reimbursements for the same procedures when performed in outpatient hospital settings. The billing patterns and flow of funds are quite complicated and so we’ve put together the diagram below to show the differences in both cases. This also shows why it can be so difficult to know the full cost of a procedure in both the hospital and ASC setting, the associated professional physician fees can vary from provider to provider even at the same facility! For this, understanding the Physician <> ASC affiliations are critical. We’ve also linked a post below going into detail regarding some of the nuance in legal structures and regulations. Given the growing number of procedures approved and loosening regulations, ASCs will likely have a large impact on the healthcare industry in the coming decade.
