Three takeaways about chronic lower back pain from Ray Baker, M.D., vice president and chief medical officer of neuromodulation, in his appearances at #DTWest2024 and on the DeviceTalks podcast: -𝐎𝐧 𝐰𝐡𝐲 𝐢𝐭’𝐬 𝐚𝐧 𝐞𝐱𝐜𝐢𝐭𝐢𝐧𝐠 𝐭𝐢𝐦𝐞 𝐟𝐨𝐫 𝐬𝐩𝐢𝐧𝐞 𝐡𝐞𝐚𝐥𝐭𝐡 🌟 Advances in technology are creating minimally invasive options for patients who want to avoid surgery. Just as importantly, better education for doctors is now allowing them to put that technology to use; and at the same time, a growing evidence base is helping patients feel confident about undergoing these procedures. -𝐎𝐧 𝐭𝐡𝐞 𝐦𝐢𝐬𝐬𝐢𝐧𝐠 𝐩𝐢𝐞𝐜𝐞 𝐨𝐟 𝐭𝐡𝐞 𝐩𝐚𝐢𝐧 𝐩𝐮𝐳𝐳𝐥𝐞 🔎 Spinal discs were long thought to be the primary cause of some back pain, yet many patients saw no benefit from disc treatment. “That left us scratching our heads,” said Ray. The resulting research led to the discovery of endplates -- bony structures attached to the discs -- as being their own source of inflammation and pain signaling. The pain produced by degenerating endplates is called vertebrogenic pain. -𝐎𝐧 𝐝𝐢𝐚𝐠𝐧𝐨𝐬𝐭𝐢𝐜 𝐢𝐦𝐩𝐫𝐨𝐯𝐞𝐦𝐞𝐧𝐭𝐬 𝐥𝐞𝐚𝐝𝐢𝐧𝐠 𝐭𝐨 𝐛𝐞𝐭𝐭𝐞𝐫 𝐫𝐞𝐬𝐮𝐥𝐭𝐬 📈 Until recently, the field “didn’t have a great way to separate out the causes of pain. That led to a trial-and-error approach” to patient care. But vertebrogenic pain has an objective biomarker that appears clearly on an MRI. Patients who receive a vertebrogenic pain diagnosis are indicated for treatment with basivertebral nerve ablation, a minimally invasive procedure proven to provide durable relief.
