Why not make palliative care available in nursing homes? Of course, this would require providing access to an interdisciplinary team that would address patients' holistic (physical, psychosocial, spiritual) needs. Support for the nursing home clinicians would only enhance care. Goals-of-care conversations would be part of the expected service. We would address realistic expectations, perhaps reducing unnecessary hospitalizations, optimizing medication regimens, and reducing polypharmacy.
