About us

CommuniCare Health Centers is a Federally Qualified Health Center providing health care to those in need since 1972. CommuniCare provides comprehensive health care services delivered by a dedicated team of providers and support staff through our clinic sites and outreach programs. Our goal is to develop and maintain a long-term, healing partnership with each of our patients, while ensuring the best possible health outcomes. Serving communities throughout the Yolo County region, CommuniCare provides health services for one in every nine residents of the area. Our services include primary medical and dental health care, behavioral health services, substance abuse treatment, health education and support services.

  • We were saddened to learn of the passing of Robin Affrime, who spent 35 years at CommuniCare Health Centers, including 19 as CEO before retiring in 2019. Robin was an absolute force and a fierce advocate at the state and national level for healthcare policy and practice. Robin oversaw tremendous growth for the services offered in Yolo County, taking CommuniCare from a tiny all-volunteer staff serving 1000 patients a year to a workforce of 300 employees serving 1 in 9 Yolo County residents. She is responsible for the introduction of dental services and the creation of sites like Salud Clinic, Hansen Family Health Center, and the Family Wellness Center - and the designation of CommuniCare as a Federally-Qualified Health Center. Her legacy lives on not only in our work as CommuniCare+OLE, but in the work of other health centers across the country. We extend our deepest condolences to her family, friends, and colleagues.     Today, our CEO Alicia Ferguson Hardy, MBA, LCSW, MSW was joined by our Chief Behavioral and Community Health Officer, Sara Gavin in Sacramento to accept the Nonprofit of the Year award for CommuniCare+OLE. We were honored to be nominated by Bill Dodd for this prestigious award.     Meet Lizbeth Guillen, a Mexican immigrant who learned English in middle school, struggled to access health care and for a time was undocumented. Today she’s a UC Davis Health resident physician who trains at CommuniCare+OLE’s Vida Family Health Center in West Sacramento where 60% of her patients are Spanish speaking. She is among the first residents from the UC Davis Department of Family and Community Medicine assigned to the teaching clinic that will soon celebrate its first year. The clinic is staffed entirely by residents and their attending physicians. Guillen feels a special bond with patients due to the common background she shares with them. “I can relate to them, because I was in their position too,” Guillen said in her clinic office. “To me, it’s a privilege to work here and make a difference in people’s lives, because I know how difficult it can be.” 💙🩺 Read more about Lizbeth's story: https://lnkd.in/g7CCapAt
