The NC General Assembly has earmarked $5 million for capacity building for Medicaid-enrolled primary care practices across the state to adopt the Collaborative Care Management (CoCM) model. In NC, physician-driven, patient-centered care benefits more than 6,000 providers, 1.6 million patients and 9 million taxpayers.
