Concordia Visiting Nurses: Professional Services - A Medicare-certified, state licensed, Medicaid approved home health agency serving Allegheny, Armstrong, Beaver, Butler, Clarion, Lawrence, Washington, and Westmoreland counties. - Our highly skilled and talented staff is comprised of: - Registered and Licensed Practical Nurses - Physical, Speech, Occupational, and Respiratory Therapists - Lymphedema Specialists - Home Health Aides - Medical Social Workers - Dietitians - Spiritual Care Advisors - Palliative Care - IV Therapy, Wound Care, Psychiatric, and Mom/Baby Specialists - Telehealth Monitoring Team - Administrative Support Team Concordia Private Care: Private Duty Services - A contracted provider with the Area Agency on Aging serving Allegheny, Armstrong, Butler, Beaver, and Westmoreland counties. - Established in 1997, Concordia Private Care has been faithfully offering top quality non-medical services including: - Assistance with activities of daily living (bathing, dressing, toileting, transfers, meal preparation, light housekeeping, and personal care) - 24 Hour Live-in Services - Personal Emergency Response Services - Medication Dispensers - Tele-CareGivers (A FREE service to place daily reassurance phone calls to the homebound patient) - Our compassionate staff is comprised of: - Personal Care Aides - Companions and Escorts - Housekeepers - Registered and Licensed Practical Nurses - Administrative Support Team - Tele-CareGiver Volunteers Respect.
