About us

Connecticut Community Care (CCC) helps people thrive at home with meaningful connection to support and services in their communities. In partnership with the State of Connecticut, CCC provides in-home care management and transitional support services to thousands of people through these programs and services: CT Home Care Program (CHCP), Personal Care Assistance Waiver, Money Follows the Person (MFP), Community First Choice (CFC), Acquired Brain Injury Program, and My Care Options. CCC provides customized, comprehensive care management services through our private care division. We have offices located in Bristol, Watertown and North Franklin. In addition, CCC leads, collaborates and makes critical connections that help foster positive outcomes for people in shared communities statewide through initiatives, The Connecticut Age Well Collaborative and The Connecticut Healthy Living Collective.
