A little bit of ischemic stroke information for the day! Learning about a post thrombectomy therapy using TNK given directly. You do have to stay with the patient for 15 minutes post thrombectomy to allow for the infusion, but as Dr. Chalouhi discussed this can really help improve getting distal thrombus. In the brain even the tiniest bit of salvage could be impactful for the patient! Have you tried this yet? One of the most interesting talks at the conference for me was a Heart Failure lecture by Dr. Juan Arnada with a segment focused on estrogens impact on heart failure! First understand the two categories of heart failure: HFpEF - Heart Failure with Preserved Ejection Fraction HFrEF - Heart Failure with Reduced Ejection Fraction This post is focusing on HFpEF Calcium Dysregulation in HFpEF: Elevated intracellular calcium during results in incomplete myocardial relaxation, increased ventricular stiffness, and elevated filling pressures. This calcium overload also activates pro-fibrotic signaling pathways, contributing to myocardial fibrosis! The decline in estrogen levels post-menopause may contribute to the higher prevalence of HFpEF in post-menopausal women, leading to increased myocardial stiffness and fibrosis. Understanding estrogen's role in calcium handling opens avenues for targeted therapies aimed at improving diastolic function and reducing fibrosis in HFpEF patients. Let’s discuss! The Edwards EVOQUE Tricuspid Valve Replacement System - minimally invasive solution for severe tricuspid regurgitation (TR). The procedure involves several key steps: 1. Vascular Access and Delivery System Insertion: A small puncture is made in the patient's groin to access the femoral vein. The delivery system, with the compressed EVOQUE valve at its tip, is advanced through the vein to the heart. 2. Valve Deployment: Once positioned within the diseased tricuspid valve, the EVOQUE valve is deployed. The self-expanding nitinol frame anchors securely within the native valve, and the integrated sealing skirt helps prevent paravalvular leakage. TRI CLIP! What is it for and how many is too many? The TriClip is a transcatheter edge-to-edge repair (TEER) system which is a minimally invasive solution for patients suffering from severe tricuspid regurgitation (TR) who are at intermediate or greater risk for open-heart surgery. In certain complex cases, deploying multiple TriClip devices may be necessary to achieve optimal results. A case study in Spain highlighted the successful simultaneous use of four clips in total to address large defect. Then there is the case in the image provided in this post with a total of 6. Starting off our recap from the Heart 2 Heart Conference hosted by #UFShands with AORTIC REGURGITATION! Why don't we have as many options for AR (aortic regurgitation) percutaneously as we do for AS (aortic stenosis)? Well aortic regurgitation causes some structural issues that make percutaneous options difficult including: - Aortic Root Dilation - Left Ventricular Dilation Surgically there is the Bentalls Procedure, where you can replace both the aortic root AND valve.
