What is the “thinker do’er” in surgical healthcare? I was asked this question by a friend? As I reflected, and paused before I jumped forward, I thought about the chapters of my neurosurgery career. In residency, in my 20’s, I was the student, learning the do. In my 30’s, I was the surgeon trying to find himself as a “do’er”. In my 40’s, I was a massive “high volume do’er”. Separately I was a “thinker”, who was too busy doing to connect the dots between do’er and thinker. After 50, after years of frustration and thinking about the problems I confronted in healthcare delivery, I started the transition to “thinker do’er”. This was the year that we opened the first JCAHO Spine Center of Excellence and opened our first Spine ASC, back in 2013. Ironically, since 2013, physicians and surgeons have progressively lost so much of the power, control and influence over healthcare delivery, indirectly arresting the development of “thinker do’er” in surgeon maturation. While I have doubled down on the thinker do’er role as I have matured as a surgeon, I want to do everything that I can to encourage healthcare professionals to exercise their influence on healthcare delivery, by becoming thinker do’ers as they grow and not do’ers who privately think. Healthcare needs our influence, our leadership and our commitment to excellence in healthcare delivery.
