About us
The Health Care Payment Learning & Action Network (HCPLAN) is a public-private partnership established to accelerate the health care system’s transition from fee-for-service (FFS) payment models to alternative payment models (APMs). This transition aims to achieve better quality care, lower costs, and healthier people. Since 2015, the LAN has shared tools and resources for designing successful APMs, measured annual APM adoption progress, and united stakeholders around a shared vision of payment reform. The LAN's goals include: -Linking 50% of all healthcare payment in the U.S. to quality and value through APMs by 2025 -Increasing alignment across public- and private-sector payers around data collection and quality measurement to reduce provider burden in APM implementation -Sharing cutting-edge APM knowledge and promising practices to accelerate the design, testing, and implementation of APMs *Reposts do not necessarily mean that the opinions expressed are endorsed by HCPLAN. #WhatDoYouValue? In #ValueBasedCare, providers work with you to understand and support your health goals so your treatment matches up with what you hope to achieve from your health care. For example, if you're trying to avoid medication, your provider may suggest other appropriate options such as diet changes or exercise. Learn more: go.cms.gov/VBC. This was the case for a retired DC chef who had diabetes. After talking with his doctor about his concerns with taking medication, he was able to lower his A1C through alternatives like healthy eating and walking. Johnson signed Medicare and Medicaid into law in 1965, there have been significant strides in the growth and improvement of these programs. For a health coach at the Johns Hopkins Brancati Center, it’s all about starting with “small changes” that her clients “are actually excited about.” The Brancati Center receives funding from the Medicare Diabetes Prevention Program (MDPP) Expanded Model to deliver a structured lifestyle intervention to prevent the onset of type 2 diabetes.
