Celebrating Healthcare Quality Week at HSNT! This week, we’re thrilled to recognize the incredible contributions of Kim Alambar and Karishma Patel to our community! Their commitment to improving patient outcomes exemplifies the heart of what we do. Karishma shared some inspiring insights on the impact of her work: "This year, I had the honor of presenting the results of our Diabetes Patient Center Medical Home (PCMH) cohort at a national symposium hosted by HRSA and the Joint Commission. During this cohort, we identified patients with very high HbA1c levels, indicating poor diabetic control. Through our value-based care approach, we saw a 46.57% improvement in the number of patients reaching diabetic control within just six months of tracking. What’s more, we also achieved our high blood pressure (HTN) control measure for the first time since tracking UDS clinical data. These outcomes go beyond just numbers—they represent improved quality of life for our patients, preventing complications from uncontrolled diabetes and hypertension. This is what healthcare quality is truly about.” Karishma highlights that quality care and well-being go hand-in-hand: "Quality care and overall well being for patients goes hand in hand, I am usually caught walking around saying 'your biggest wealth is your health!' Quality of care is impactful not only when we speak of when a patient is 'sick' or has chronic conditions such as diabetes or high blood pressure but one of the biggest impacts on quality care is preventative medicine. Seeing the impact of preventing patients from experiencing poor prognosis by early detection or educating patients on preventive care will always help patients and their families live healthier more fulfilling lives thus creating healthier communities not only in the communities we serve at HSNT but all across our nation and that really is the impact of health centers nationwide!" Karishma emphasizes that walking is one of the most underrated ways to improve your health. Taking just 20 minutes a day for a walk can have a powerful impact on your well-being.
