This week, Health Share was pleased to announce $11 million in grants awarded to 38 Community-based Organizations (CBOs), of which two-thirds are culturally specific. These Community Capacity Building Funds (CCBF) will go towards supporting new housing and nutrition benefits through the Oregon Health Plan. These CBOs will be instrumental in delivering Oregon’s groundbreaking Health-Related Social Needs* (HRSN) benefits, which include supports for climate, housing and nutrition. There will be another opportunity to apply for CCBF in 2025, check the website** for updates. The topic was improving access to social-emotional health services for children birth-age 5, and a full house of brilliant cross-sector partners participated, bringing their considerable passion and wisdom to bear. Health Share is closed today in observance of the 159th anniversary of Juneteenth, the date in 1865 when the final enforcement of the Emancipation Proclamation was ordered in Texas at the end of the American Civil War. We hope you and yours have time to reflect today on both the progress we've made since then and the tremendous amount of work that remains to be done. At Health Share of Oregon, we are working towards not only closing health equity gaps for our Black and African American members but ensuring that members of these and other BIPOC communities play a meaningful role in shaping our policies, priorities and investment strategies. Today was a pretty remarkable day. Health Share of Oregon welcomed representatives from Clackamas County, Washington County, Multnomah County homeless services and Metro alongside our local healthcare systems: CareOregon, PacificSource Health Plans, Oregon Health & Science University, Kaiser Permanente, Providence Health & Services and Trillium Community Health Plan. Community Solutions, Inc. engaged our local healthcare and homelessness systems, looking at what we have accomplished together so far, and dreaming big about the future of our integration work. There’s a long way to go, but anything is possible with the collaborative partnerships that we are developing. I am energized and inspired by the work we are doing and what’s ahead.

Health Share of Oregon's Director of Community Health, Maria T., presents at the first in-person Traditional Advisory Committee meeting of the post-pandemic era. Focused on promoting successful integration and utilization of Traditional Health Workers (THW's), including doulas, peer support and wellness specialists, personal health navigators and community health workers, the committee seeks to increase access to these providers, who play a critical role in advancing health equity. Working in community, the Advisory Commitee identified key deliverables related to THW infrastructure, such as payment models, referral systems, tracking and reporting- tools that help us deliver on the vision of connecting our members to THW's of their choosing, in settings where they can thrive. Held at Asian Health & Service Center, the meeting kicked off with food and traditional music performed by BOD and CAC member, Candice Jimenez and CAC member Hilary Flaming and included a blanket ceremony honoring outgoing board member, Camille Applin-Jones, MBA, BSN, FACHE. The group engaged in important discussions regarding investments in #socialdeterminantsofhealth and priorities for Health Share’s upcoming Community Health Improvement Plan. Members of both committees felt inspired by the positive energy and spirit of collaboration the gathering fostered and looked forward to working together more closely moving forward. #membercentered #community #medicaid #healthequity

Health Share of Oregon is honored to have partnered with Juntos NW and Fresh Out Community Reentry Program in sponsoring the 2nd Annual Black, Brown and Indigenous Conference at Victory Outreach Church. The conference, which was free to all attendees, provided a great opportunity for both communal learning and celebration. Health Share is committed to forging partnerships with Black, Brown and Indigenous communities and humbly recognizes that listening is a crucial first step in the process. At the conference our Health Equity and Engagement Team heard first-hand perspectives on community health needs and related impacts - input that will guide us as we create community health and engagement strategies moving forward. #healthequity #communityhealth #leadingwithrace

Founded on the idea that Oregon Health Plan recipients benefit when health plans work together, we’ve spent the last dozen years collaborating, innovating, investing, and always putting members at the center of what we do. We've accomplished so much and yet, so much remains to be done. Our collaborative is facing some of the toughest challenges in a generation, but we remain steadfast in our commitment to continuous improvement, advocating for those most in need and breaking down the barriers that stand in the way of health equity. It may not be how most spend their teen years, but at Health Share it's who we are and what we do.
