It is supported by a coordinated clinical-service-care team built for value-based care capable of managing appropriate acuity situations safely outside an institution including in-person care. HLTHX effectively drives care to members —into their homes, their workspace, and their travels – creating a continuous relationship to meet healthcare needs when, where, and how members, their families, and caregivers want to connect. HLTHX achieves a continuous long-term relationship with members and their families rather than the fragmented, episodic, and transactional interactions of conventional care.

Jessica has a background in the Operating Room, PICC lines, Infection Control and Care Management. She has been invited to unique ESRD/ Renal program and SNP/ Special needs programs, respectively, to guide members with polychronic diagnoses to meet individualized goals. She has received several awards for Care management, training modules and guidance to her peers. Jessica has contributed her knowledge to the health start up world and has launched several successful startup health companies to date.
