The mission of the National Institute on Aging (NIA) IMbedded Pragmatic Alzheimers disease (AD) and AD Related Dementias (AD/ADRD) Clinical Trials (IMPACT) Collaboratory is build the nation’s capacity to conduct pragmatic clinical trials of interventions embedded within health care systems for people living with dementia and their caregivers. The vision of the IMPACT Collaboratory is to transform the delivery, quality, and outcomes of care provided to Americans living with dementia and their caregivers by accelerating the testing and adoption of evidence-based interventions within healthcare systems. We accomplish this mission by: -Developing and disseminating best practice research methods -Supporting the design and conduct of pragmatic trials, including pilot studies -Building investigator capacity through training and knowledge generation -Catalyzing collaboration among stakeholders, healthcare providers, and investigators -Ensuring the research includes culturally-tailored interventions and people from diverse and under-represented backgrounds

📢 New Study on Goal-Concordant Care in Nursing Homes Members of the NIA IMPACT Collaboratory including: Natalie Ernecoff, Ellen McCreedy from Brown University School of Public Health, Laura Hanson of the University of North Carolina at Chapel Hill, and Susan Mitchell from Hebrew SeniorLife and Hyunkyung Yulia Yun from Brown University have tested a pragmatic approach to identify goal-concordant care for nursing home residents with Alzheimer’s Disease or Related Dementias (ADRD). The study used NH EHR data to operationalize a feasible pragmatic approach to identify NH residents with moderate to advanced ADRD who received goal-concordant care. The findings indicate that 87% of residents received such care, however, goal-discordant care was not rare. The study supports the feasibility of the approach, but identified limitations including: (1) goal determination was limited to orders, and did not include more nuanced discussions documented in progress notes or other text; (2) changes in preferences that may have occurred for in-the-moment decisions could not be captured; (3) other goals of care (eg, prioritization of maintaining function) could not be captured; and (4) goal-discordant care for residents whose goal is life prolongation would be challenging to operationalize using this approach. This approach may inform outcome selection in future pragmatic trials evaluating the effectiveness of interventions that aim to improve the concordance of care with goals among NH residents with a preference for comfort. 👉 Read the full article: https://bit.ly/4fgB9lM #Alzheimers #Dementia #GoalConcordantCare #NursingHomes
