There seems to be a reluctance to treat patients with medications that entail taking blood levels or dosage monitoring, extra patient education, attentiveness to interactions and side effects, or a step outside a comfort zone. Think there’s nothing to worry about when it comes to #aripiprazole and the #QT interval? In our case report / lit review published today in Journal of Clinical Psychopharmacology, we (Dr Martin Osugo, Dr Antonio de Marvao, Prof Oliver Howes) describe a concerning increase in #QTc duration (from 420ms to 560ms!) in a healthy male taking aripiprazole 5-10mg daily as part of a #neuroimaging study 👇 https://lnkd.in/e5zJ_B2Z There are a few other reports of QT prolongation on aripiprazole; however, the link between drug and side-effect is clearest in our case - 1️⃣ our subject was a healthy volunteer on no other medication 2️⃣ ECGs at other time points were normal, as were blood tests, vital signs and clinical exams throughout the study 3️⃣ we collected plasma levels of drug and metabolite, so we *know* when the subject was and wasn’t taking the drug Finally - and perhaps most interestingly 🤔 - we sent the subject for clinical and genetic tests for #congenital #LQTS. Whilst most tests were normal, he was found to have a rare variant in one copy of the #Kv11.1 gene 🧬, which is thought to be a risk factor for QT prolongation under some conditions (such as certain drug exposure 💊 …) We call for a “thorough QT” (#TQT) study of aripiprazole, as has been undertaken for some other #antipsychotics (and many other non-#psychotropic medications). There are few reports of genitourinary side effects with atomoxetine. A new case report in JCP describes a case of painful ejaculation with this medication.
