Our Operations Lead Team volunteered at the Howard County Food Bank on Wednesday, sorting and packing food donations, stocking shelves and distributing food to families in need. We value opportunities like this to be out in the community, contributing in a hands-on way to one of our organization’s key priorities: access to healthy food.

We created a proactive, segmentation-driven approach that aligns strategies & resources, improves outcomes, and reduces costs across diverse populations. Kristin’s team has positive early results: 💡Updated care management identification approach, targeting patients with 𝙥𝙧𝙚𝙙𝙞𝙘𝙩𝙚𝙙 𝙝𝙞𝙜𝙝 𝙘𝙤𝙨𝙩 and 𝙙𝙚𝙛𝙞𝙣𝙚𝙙 𝙤𝙥𝙥𝙤𝙧𝙩𝙪𝙣𝙞𝙩𝙮 𝙩𝙤 𝙞𝙢𝙥𝙧𝙤𝙫𝙚 𝙘𝙖𝙧𝙚 📊 𝙎𝙩𝙧𝙚𝙖𝙢𝙡𝙞𝙣𝙚𝙙 𝙖𝙥𝙥𝙧𝙤𝙖𝙘𝙝 𝙩𝙤 𝙙𝙞𝙖𝙗𝙚𝙩𝙚𝙨 𝙤𝙪𝙩𝙧𝙚𝙖𝙘𝙝 generated 18% improvement in A1C control and 21% increase in blood pressure control measures 🤰 High Risk Maternity Program increased postpartum visits in 𝙝𝙞𝙜𝙝-𝙧𝙞𝙨𝙠 𝙈𝙚𝙙𝙞𝙘𝙖𝙞𝙙 patients 🎯 By re-deploying staff and optimizing vendor relationships, the team focused their efforts to optimize impact AND eliminated redundant outreaches The impact of this structured, proactive approach has been phenomenal, and I’m excited to continue our collaboration over the coming year!
