According to our survey of state Medicaid directors, reduced revenue collections, expiration of pandemic-era federal funding, and other macroeconomic uncertainties may discourage states from increasing funding for access to behavioral health, long-term care, and higher provider reimbursement rates — and could prompt spending reductions. In states where abortion is banned or restricted to the first six weeks of pregnancy, women with young children are more likely to say it is difficult to obtain key services in their state to support them and their families — up to double the rate of those who live in states with few or no abortion restrictions. Our new analysis finds that state Medicaid officials expect the program’s national enrollment to decline by about 4% and state Medicaid spending to rise by 7% in fiscal year 2025. These rates follow a larger, but anticipated, enrollment decline and state spending increase in fiscal year 2024, as pandemic-era policies and federal funding expired. While state fiscal conditions remain stable heading into fiscal year 2025, the longer-term fiscal and policy outlook for Medicaid programs is less certain. Get the details in our analysis:

Our 2024 Women’s Health Survey provides new data on access to fertility care — including women’s opinions about access in their state, cost barriers, and the range of fertility services that women use. We find 1 in 8 reproductive-age women say they or their partners needed fertility assistance to become pregnant or prevent a miscarriage. We also find that 12% of reproductive-age women who said they needed fertility services didn’t receive care — primarily because of cost. Our 2024 Women’s Health Survey finds that women aren’t fully aware that the Affordable Care Act requires insurance plans to cover the full cost of recommended preventive health care services — especially contraception. While most women are aware that their health plan must cover the full costs of annual check-ups (71%) and routine mammograms (73%), significantly fewer are aware of contraceptive coverage requirements (43%).
