If you are passionate about making a difference, consider joining our team! -Care Manager, Behavioral Health for Albemarle (R9355) This mobile position involves planning, coordinating, tracking, and monitoring care for individuals to help them achieve key goals under the BH I/DD Tailored Plan. You will actively engage in care coordination, health promotion, and transitional care, supporting individuals in making informed choices. -Care Manager, I/DD (Travel between Albemarle and Monroe) (R9268) As a Care Manager specializing in I/DD, you will coordinate care team meetings and referrals to community resources, empowering individuals to manage their own healthcare.     Today marks the start of National Case Management Week! At Monarch, we’re honoring our incredible team of four Directors, 17 Care Managers, and 11 Care Manager Extenders. Join us in celebrating their hard work and dedication this week! These dedicated professionals plan, coordinate, and monitor care for people with behavioral health, substance use, and intellectual and developmental disabilities. They guide individuals through transitions, organize care team meetings, and link them to vital community resources, all while using trauma-informed, person-centered practices.
