Monogram’s Aashish Shah was asked to talk at Healthcare Innovation about how our multispecialty provider clinical model is leading innovation in the SDoH space for complex kidney patients. Across the 150,000 patients that Monogram cares for nearly 2/3 are people of color and minorities and over 3/4 live below the US median household income level. They overwhelming have limited income and are reliant on government programs like Medicare and Medicaid. Not surprisingly the most common burdens center around housing, food, and transportation insecurity. Every month Monogram closes 40% of the SDoH burdens it encounters. We’re proud of the help we provide but there is more to do. Joining in the panel discussion was KATHERINE LUSK, CEO of Texas Health Services Authority (Statewide HIE) and Mark Hagland of Healthcare Innovation. Together they shared how pairing with notable payers and partners like Memorial Hermann Health System has expanded their collective ability to help those most in need.
