Modern Healthcare: Calls for Family #Caregiver Support Escalate as Home Care Grows https://lnkd.in/dirN4Ag6 Diane Eastabrook The role of unpaid family caregivers is drawing focus as Congress decides the future of a hospital care at home waiver. The role of family caregiving extends beyond hospital-at-home programs as a growing number of older adults are aging in place. Moving Health Home, a trade group representing home-based care providers, said in an email that caregivers are central to all models of home care which include home-based primary care, skilled nursing and therapy. All of the models the group is advancing take into account caregiver training, emotional well-being and necessary support, said Moving Health Home founder Krista Drobac in the email.

Hospitals face inpatient bed capacity constraints from extended stays, staffing shortages and overcrowding. The Wall Street Journal: Hospitals’ New Push: Treating Patients in Their Homes Melanie Evans https://lnkd.in/daBvfSFC Around the country, more than 300 hospitals are deploying or preparing to dispatch paramedics, nurse practitioners and other medical staff to treat patients at home instead of in hospital settings, a service widely referred to as #hospitalathome. The idea outlasted the public health crisis and hospital finances rebounded, attracting investment from medical centers and technology companies eager to capture a piece of the emerging market. #Medicare won’t pay for #athome care after December, unless Congress votes to continue the program. Despite this, home services have grown under the federal insurer for the elderly and disabled. Some commercial insurers, including Medicare Advantage, also cover hospital-at-home services, according to hospitals and insurers. “Who doesn’t want to be home?” said Asere, 71, a retired nurse. Medical staff check on patients overnight, interrupting their sleep, she said. “It’s not a restful place.”

Push for hospital-at-home waiver extension increases Modern Healthcare Diane Eastabrook The Bipartisan Policy Center released a report, recommending Congress to extend a waiver program allowing hospitals to provide #acutecareathome. It also recommended #Congress direct Centers for Medicare & Medicaid Services to strengthen regulatory guidance, such as on safety protocols related to fall prevention, infection control, #telehealth and #remotepatientmonitoring. Moving Health Home, a trade group representing hospitals and #hospitalathome vendors, said the center's recommendations were reasonable and that standardized #quality measures are appropriate and essential to scaling the care model. https://lnkd.in/gnMU5b-e

The Bipartisan Policy Center released a brief on the future of acute hospital care at home. The report, released today, provides immediate-term federal policy reform recommendations. Recommendations focus on collecting data collection and research, facilitating the appropriate and diverse uptake in the model to inform any future reauthorizations and more. STARTING NOW: The FDA is holding a stakeholder listening session to discuss the home environment as an integral part of the health care system, including hearing stakeholder ideas on aspects to consider in developing the #Home as a Health Care Hub prototype for all people in the U.S. FDA Commissioner Robert Califf provided opening remarks, emphasizing the importance of home. The FDA is using #diabetes as an example health condition for the prototypes given the impacts over the lifecycle of someone living with this condition.  The Home as a Health Care Hub prototypes are the beginning of the conversation—helping device developers consider novel design approaches, aiding providers to consider opportunities to educate #patients and extend care options, generating discussions on #valuebasedcare paradigms, and opening opportunities to bring #clinicaltrials and other evidence generation processes to #underrepresented communities through the home. https://lnkd.in/gu7q4DUk

Hospital-at-home technology and a small number of health systems offer #homebased skilled nursing programs as a way to move patients more quickly into post-acute care, cut costs and reduce hospital readmissions. “[Skilled nursing-at-home] is urgent because we have so many patients in hospital beds that don’t need to be there,” said Krista Drobac, Moving Health Home's founder. Traditional fee-for-service Medicare won’t reimburse for the service and only a handful of Medicare Advantage plans and commercial plans will cover it through negotiated value-based contracts. Moving Health Home member Inbound Health, for example, negotiates contracts that reimburse at a fixed rate that is lower than the cost of a 30-day episode of care at a skilled nursing facility. “It is essentially a rate that is traditionally somewhere between a traditional home health episode and what a traditional brick-and-mortar [skilled nursing] episode is,” said Inbound CEO Dave Kerwar.
