Google “snacks for people with diabetes” and you will see a lot of articles and ideas about what to eat. Roasted chickpeas, almonds, air-popped popcorn, and chia seed pudding are healthy suggestions. But you may also find recommendations for highly processed foods like beef sticks and other foods high in saturated fat and sodium. So, how do patients decide on snacks that will truly help control diabetes and their overall wellness? Have them talk to a Registered Dietitian Nutritionist (RDN) who will work with them to create a personalized food plan. My Diabetes Tutor’s Medical Nutrition Program’s RDNs help people with diabetes select foods, including snacks, that are tailored to individual health goals and personal preferences. Education and support that's proven to help manage diabetes, reduce A1C, and improve overall health.
About us
Why was My Diabetes Tutor founded? Dr. Prem Sahasranam, who goes by Dr. Sahas, is a Board-Certified Endocrinologist and the Founder of My Diabetes Tutor. He founded Central Valley Endocrinology in 2007, a state-of-the-art endocrine practice in Central California that provides exceptional care for adults in the rural Hanford-Visalia area which serves half a million people. He discovered that diabetes occurs at higher rates in rural versus urban areas, and people living in these rural areas have more diabetes-related complications. This scenario motivated Dr. Sahas to establish My Diabetes Tutor, Inc. (MDT) in September 2020 to bridge the gap in diabetes education, because with appropriate self-management education and support, people with diabetes can learn how to manage their diabetes, thereby preventing such complications. What does My Diabetes Tutor do? MDT is a nationally accredited, 1:1 Diabetes Self-Management Education and Support (DSMES) telehealth solution. It is the first program to provide in-patient diabetes education on-call services. It collaborates with health plans, health systems, and providers to deliver coaching to patients in the privacy of their own homes or via health provider kiosks. The program also has a diabetes prevention program, remote patient monitoring, and weight management program under development. Is it working? MDT outcomes have outpaced the national average for DSMES, delivering an average 1.6 A1C reduction for patients. Is MDT covered by insurance? MDT is 100% covered by Medicare and covered by most insurance providers in California. What does the future look like for MDT? After showing positive outcomes in rural Central California, MDT is expanding to address a great need to make diabetes education accessible in other rural communities. Do your patients need help determining how to use a continuous glucose monitor (CGM)? My Diabetes Tutor (MDT) can help! MDT offers virtual education sessions that guide patients to the CGM that may be best for them. My Diabetes Tutor’s (MDT) study on effectively reducing health disparities showed that an All-Spanish Diabetes Self-Management Education and Support (DSMES) curriculum reduced A1Cs by 1.6% and helped achieve healthy eating goals for 57% of participants. Given the absence of Spanish-language curriculums for online delivery, MDT’s tailored program addresses this vital gap. Our nationally accredited DSMES program delivers in a culturally appropriate way and includes after-hours availability and telehealth methods. Does the order in which you eat your food during a meal impact your blood sugar? Research says yes, and several studies have found that eating vegetables and protein before consuming simple carbohydrates had a beneficial impact on post-meal blood sugar. My Diabetes Tutor’s Medical Nutrition Program helps patients understand the role of food on blood sugar. Our nutrition specialists work with each patient to create an eating plan that supports managing the condition. Knowing what percent of the time blood glucose levels are “in range” offers big benefits in managing diabetes. Time in range (TIR) data helps patients understand the impact of food, activity, illness, medications, and other factors on blood glucose. The easiest and most accurate way to calculate TIR is using a continuous glucose monitor (CGM). Wearable CGM technology tracks glucose levels over time and measures the glucose level in the interstitial fluid 24 hours a day My Diabetes Tutor’s (MDT) Certified Diabetes Care & Education Specialists (CDCES) offer virtual device training to help patients set up CGMs and sync them to smartphone apps. They explain how to understand and respond to the data and provide problem-solving advice for this transformative technology.
