About us
Since 2011, we have provided strategic planning and training to aid fulling missions and challenges related to providing the best care for patients and their care givers. Our work in population health, care coordination, and related value-based initiatives has allowed us to establish a professional presence in over 18 states. We are proud to have reduced ED visits and hospital readmissions while improving preventive care gaps and patient access. Nautical Health Concepts also provides training for organizations and professionals.
