Welcome to OFMQ! For more than 50 years, the Oklahoma Foundation for Medical Quality has been a trailblazer in quality improvement. Founded in 1972, OFMQ was chartered as a non-profit organization of licensed medical professionals to oversee peer review initiatives across the Sooner state. Today, though we maintain the medical review expertise foundational to our business, OFMQ has expanded to become Oklahoma’s premier partner for quality improvement resources. OFMQ has worked with 80% of the 300+ nursing homes in Oklahoma since 2002. We have over 20 years of experience in providing direct technical assistance to LTC facilities and over 40 years of experience in helping hospitals improve various clinical areas as well as report quality data to CMS. In current OFMQ projects, we have helped LTC facilities to: Achieve a 26.7% Relative Improvement Rate (RIR) in reducing falls with major injury (76.1% of facilities experienced improvement); 27% RIR in reducing pressure ulcers (68.2% of facilities improved).

The OkDCN is a collaborative statewide program to improve the care and health outcomes of older adults living with Alzheimer’s disease and other dementias and their family and friend caregivers.
