New research shows states that grant full practice authority to nurse practitioners reduce total healthcare costs for people with diabetes by a whopping 20% in urban areas! And in rural areas, full practice authority drives down usage of advanced medical services by people with diabetes by about 10%. The study also found that full practice authority had no effect on primary care claims, the number of high blood-sugar tests, or diabetic debridements, which are common procedures for people with diabetes. Ending burdensome collaborative agreements and allowing NPs to practice independently would also let doctors reserve more of their time for the complex clinical situations they have been uniquely trained for. There’s no good reason to preserve outmoded scope-of-practice laws that artificially constrain the supply of health care — and hurt patients and drive up costs in the process. PCNP is the state organization that promotes and protects the practice of over 10,700 Certified Registered Nurse Practitioners (CRNPs) in Pennsylvania. Formed in the 1980s by only three forward-thinking NPs, PCNP now has 18 regional groups across the Commonwealth. Each regional group offers networking opportunities and has a representative on the PCNP Executive Board that meets quarterly to discuss issues and set organizational goals. In other words, states can dramatically reduce health care costs without sacrificing the quality of care simply by deregulating their health care labor markets. And it doesn't cost a dime of taxpayer money. Obviously, some advanced diagnostic and treatment decisions and procedures can only be handled by doctors. But NPs undergo rigorous training programs that enable them to provide most primary care services — and identify when a consultation with a medical doctor is advisable. NPs are just as adept as doctors at diagnosing common diseases and prescribing widely used drugs. It's time for Pennsylvania to grant our NPs full practice authority and make it easier for patients to access high-quality health care. Pennsylvania is losing nurse practitioners to neighboring states that already offer NPs full practice authority. These NPs are leaving to do the job that they've been trained and educated to do, without the unnecessary restrictions placed on their ability to practice and care for patients. Studies show that full practice authority laws increase the number of NPs who remain and work in the state after graduation. And once practicing, NPs are more likely to move to a state with full practice authority, rather than the reverse. This means Pennsylvania likely won't gain back NPs who have moved to other states – unless PA enacts full practice authority legislation. In the midst of severe healthcare workforce shortages, Pennsylvania should be taking action to become a desirable place for NPs and other skilled healthcare professionals to work, rather than restricting their ability to practice. Full practice authority can help retain our valuable NP workforce, gain back NPs who left due to restrictive laws, and attract talented NPs from other states. And a greater number of NPs means greater healthcare options and access for patients across Pennsylvania. Nurse practitioners in the VA health system have full practice authority and are trusted healthcare professionals who care for our country's veterans and their families. The Veterans Health Administration is one of the largest integrated health care systems in the United States, employing thousands of NPs across VA medical centers and community‐based outpatient clinics across the country. A 2020 study comparing MD-assigned patients to NP-assigned patients in the VA health system found similar results in costs, clinical outcomes, and receipt of diagnostic tests across both patient groups. The study also found that the NP patients had less hospitalizations overall, as well as comparable outcomes across differing levels of patient comorbidity. These results suggest NPs may be effective at reducing patient hospitalizations and show how primary care NPs don't need to be limited to caring for less complex patients. The study adds to the ongoing decades evidence showing that NPs provide effective, high-quality health care and are a cost‐effective approach to addressing primary care shortages across the country.
