Working hard in Kentucky to fulfill our mission: to improve the health and quality of life of our members.
About us
Since 1997, Passport Health Plan has worked closely with local providers and community advocates to improve health outcomes for the Medicaid population. Our plan has made a huge difference for our members and won us a place as the top Medicaid plan in Kentucky by the National Committee for Quality Assurance (NCQA) for 2016-17. Currently, we serve more than 320,000 members. Passport is supported by Molina Healthcare, founded in 1980 with a mission to provide quality health care to those who need it, no matter their circumstances. Today, Molina health plans serve 3,331,000 members across the country through government-funded programs. Each day, we work to earn the trust our partners and members put in us, so that can lean on Molina. Together, Passport Health Plan and Molina share a commitment to improving the health and quality of life of our members across the Commonwealth of Kentucky.
