About us

Pathways is a Bay Area not-for-profit organization providing hospice, home health, and palliative care services. Founded in 1977, Pathways serves more than 5,000 families annually in five Bay Area counties. We care for patients in their place of residence whether it be their home, hospital, nursing home or assisted living facility. Affiliated with El Camino Hospital and Sequoia Hospital since 1986, Pathways Home Health & Hospice is accredited by ACHC. It is a member of the Visiting Nurses Association of America, California Hospice & Palliative Care Association, and the National Hospice & Palliative Care Organization.

  • Our nurses and therapists are frequently at the forefront of our important work in health services and are the backbone for our shared mission: committed care for your loved ones and your families in their time of need.  Kara is an experienced Pathways bereavement counselor with over 20 years of experience working within mental health and wellness related industries. Kara revealed some lesser known observations during counseling sessions, “I once had a woman enter my care who had suffered a tragic loss as one of her twin children passed during childbirth.”  The client and her husband had hit a roadblock with their shared grief. Kara noticed something about the way they described their pain. They both realized that the simple act of speaking the chosen name of their lost child was something they subconsciously refused to do. The realization that they were not confronting the reality of the life they had lost hit home in a healing way. Once they allowed themselves to include his name in their conversations about the impact of the loss they both profoundly felt, they were able to identify how the loss had affected their lives and were finally able to begin the arduous process of building beyond their grief. “Bereavement is more than just caring for families during a time of grief. I see many people with a lot of baggage that grief has compounded on, causing an ‘iceberg’ effect. Many of my patients may have never spoken to a counselor or a therapist. Honey, another nurse serving our East Bay region, shares challenges with communicating important information to patients and their families with a language barrier, “As an RN case manager, I ask our families about their expectations and knowledge of hospice care. I speak Spanish, and oftentimes I can give Hispanic families a sense of trust when communicating the needs of my client to them in Spanish.” Communicating medical information in a patient's native language is essential for ensuring accurate comprehension, fostering trust and respect, addressing cultural beliefs and practices, and ultimately improving patient satisfaction and health outcomes. By prioritizing clear and culturally sensitive communication, healthcare providers can bridge language barriers and empower patients to actively participate in their care, leading to a more equitable and effective healthcare system for all. “As a case manager, I have to work to solve problems. Knowing I am helping these patients, making them feel good, and to see them happy with a smile, you know they are content with your care.
