We offer our patients personalized preventative care that is delivered and coordinated within the community they live in. Our providers collaborate with social workers and organizations to better outcomes through improving social determinants of health (SDOH). Our goal is to help seniors stay healthy at home. We deliver a different, more compassionate model of primary care – one that connects with patients where they live through in-person and telehealth.

After just two months of practicing medicine at the Owl's Nest senior community in East Cleveland, we have seen some amazing results. Nearly 50% of our residents have utilized our services, which include primary care, condition management, GYN, labs/prescriptions, screenings/vaccinations, and care coordination. And our NPS is exceptional -- scoring in the 90's. Our team of compassionate, highly-trained providers are hitting it out of the park!
