Given the ongoing shortage of IV fluids in the US, this paper provides a timely and nice review of the literature on choice of fluids and approaches to fluid administration in the ICU. There are risks that both over- and under-resuscitation can lead to poor outcomes in critically ill patients. It is therefore reasonable to consider using advanced hemodynamic monitoring to guide fluid therapy. There is a shortage of IV fluids: damage to Baxter’s manufacturing facility from Hurricane Helene has caused a shutdown of Baxter’s manufacturing facility. Reports suggest Baxter provides up to 60% of domestic IV fluid supplies. Other manufacturers are increasing production, but it will take some time. Careful management of fluid administration and hemodynamics is more important than ever. If you’re expecting to have to be more restrictive with fluids, advanced hemodynamic monitoring can help you ensure patient safety. Retia Medical’s Argos monitors do not require expensive disposables and can be used as often as needed. You can round with them in the ICU to check on the fluid status of any patient. “It takes 20 years to build a reputation and five minutes to ruin it. If you think about that, you’ll do things differently.” - Warren Buffett Integrity and Service are two of Retia Medical’s core values. Of course, I talk about our core values during company meetings. I and our executive team also do our best to model these behaviors. Yet, you wonder whether it makes a difference. A few weeks ago, one of our clinical managers, Rosemarie Woodstock, was on a plane and happened to sit next to another nurse. They get to talking about their respective careers and then this other nurse saw Rose’s backpack. She recognized the company name and started talking enthusiastically about how they love our Argos hemodynamic monitor and how much they rely on it for accurate data. Just as importantly, they were so appreciative that we had come into their facility on short notice to help train some of their new nurses on the Argos to ensure compliance prior to an upcoming visit from the Joint Commission. I emphasize to our team that when they exhibit behaviors that reflect our core values, it comes back to us. Here is one small example. 📣 We are thrilled to announce the findings of a recent study that evaluated the accuracy of our Argos multi-beat analysis against the PiCCO pulse-contour analysis during a passive leg raise (PLR) and/or a fluid challenge (FC). 📣 In this study Dr. Bitker's group found that the Argos monitor has an excellent trending ability for cardiac output monitoring compared to the PiCCO thermodilution method, with a concordance of 90%. In 29 MICU patients with sepsis and septic shock, the Argos monitor was able to predict fluid responsiveness when undergoing a passive leg raise. Likewise, the Argos monitor correctly tracked fluid responders on receiving a fluid challenge. The article concludes that "the device [Argos] showed excellent diagnostic and predictive performance to predict preload dependence in a medical ICU population receiving norepinephrine". https://lnkd.in/eg9bmudW
