About us
Roots Community Health is a 501(c)3 non-profit organization dedicated to reducing health disparities in East Oakland, California. Roots is the first community health center in East Oakland to provide physician care at patient homes and residential facilities. Roots Community Health Center celebrated the opening of its new facility at 9925 International Blvd. in March of 2011. Our mission The mission of Roots Community Health Center is to uplift those impacted by systematic inequities and poverty by providing culturally responsive, comprehensive healthcare, behavioral health, and wraparound services; identifying and addressing root causes of illness and suffering; and emphasizing self-sufficiency and community empowerment. Roots Community Health Center accomplishes its mission by providing top-quality healthcare, conducting community-based participatory research, and offering opportunities for rehabilitation, education, training, and employment to reduce poverty and dependency in our community. Roots implements its programs and services while honoring the “roots” of culture, heritage, and tradition; by providing unprecedented access to services; by remaining community-aware and community-responsive; and by establishing partnerships to ensure a more efficient continuum of care in Oakland.

How can we improve health literacy for underserved Black communities throughout #Oakland and the #BayArea? This month is #HealthLiteracyMonth! Since being founded in 2008, Roots Community Health has done so much for the community and a key focus of Roots is health literacy—empowering individuals with the knowledge they need to make informed health decisions! Through medical and behavioral health care, nutrition, wellness, education, and weekly public health updates, we ensure equitable access to health education, by breaking down complex health info into simple terms. As we also celebrate #HealthEducationWeek, we invite everyone to join the conversation about the importance of clear health information. #RootsEmpowers #CommunityHealth #HealthEquity #NHEW24

October is Infant and Pregnancy Loss Awareness Month, a time to remember the little ones we lost too soon. We also take a moment to acknowledge Black mothers, who have a maternal mortality rate more than three times higher than non-Hispanic white women in the U.S. Roots Community Health continues to help eliminate the racial disparities in maternal and infant mortality through our Doula and Women of Wellness Programs, offering services for FREE from 27 weeks pregnant to 1 year postpartum! Let’s break the silence together and uplift our community. If you or a loved one needs support, reach out to us! You are not alone on this journey. #RootsEmpowers #InfantLoss #PregnancyLoss #BlackMaternalHealth #BayArea

Did you know distrust in the medical system can lead to poor health outcomes? For people of African ancestry, regular medical check-ups and a healthy lifestyle are critical in preventing #Alzheimers, #dementia, and other conditions like heart disease, high blood pressure, and diabetes that increase our risk. There are many benefits of regular check-ups, yet when speaking to a healthcare professional or visiting a healthcare setting, we understand how important to feel, seen, heard, and respected! Don't be afraid to express your concerns and ask questions during your doctor’s visits. If you’re unsure where to go, seek out health establishments like Roots Community Health, that will make it a point to give you the type of care you need and deserve. Use the QR codes and links for more resources, share this with others, and stay tuned for more information, as we work together to #mindyourbrain for a healthier future! #HealthyBrain #RootsEmpowers #HBI #HealthyAging #HealthyBrainInitiative #PreventativeCare
