Leveraging federal funding, the ARKANSAS RURAL HEALTH PARTNERSHIP assisted seven member hospitals in the southeast Delta Region to implement an evidence-supported Critical Access Hospital transitional care model. The model offered Medicare beneficiaries a local post-acute care recovery option and participating hospitals experienced positive financial results, along with positive patient satisfaction surveys.
