Black/African-American individuals are nearly 4 times more likely to develop end stage kidney disease (ESKD), and Hispanic and Native American individuals are more than twice as likely to develop ESKD compared with White individuals.* Somatus’ integrated, whole-person care delivery model is making a meaningful impact on promoting health equity. Through our innovative, in-home clinical model, Black/African-American patients participating in the Somatus program have: - 63% higher rates of access to nephrology care. - 4.1x higher adoption of disease-delaying medication utilization. - 95% higher rates of home dialysis starts. - 3.4x higher pre-emptive kidney transplant waitlist rates. All outcomes are compared to the national averages.
