The Spear Scholarship serves to uplift and support the next generation of clinicians. It was an honor to meet and talk to the 2024 recipients about their goals and vision for the future. In addition, we were thrilled to unveil a commemorative plaque, symbolizing the deep bond between our organizations. It was a heartfelt homecoming for our co-founders, Dan Rootenberg and Dave Endres, who reconnected with their alma mater and gave back to the institution that helped launch Spear. A big thank you to School of Health Sciences of Touro University for their warm welcome. I had the pleasure of co-leading a Special Topics Seminar on Return to Sport testing for upper extremity injuries back in July. The current, most well-known protocols for return to sport testing for lower extremity injuries are insufficient. For upper extremity injuries, they’re almost non-existent. Return to sport testing for upper extremity injuries is a broad topic that varies significantly based on the individual, the injury, and the demands of the sport. With that said, there are three key principles that should be applied when assessing sport readiness for any athlete recovering from an upper extremity injury: 1. The recovery and return to play for athletes after injury is a multifaceted clinical decision that occurs as an ongoing process that exists on a continuum. Returning to sport isn’t a one-time, one-person decision. The decision is made on a continuum that evaluates athlete readiness before, in conjunction with, and after an athlete returns to the field for their sport. 2. Return to sport/return to performance requires a battery of tests to determine physical and psychological readiness. Strength tests, upper quarter Y-balance tests, and med ball throw tests are useful tests, but they are not good enough on their own to determine readiness. Assess the athlete’s physical (strength, ROM, power), neuromuscular (motor control, movement patterns, neurocognition), and psychological (confidence, fear-avoidance) readiness to return to practice. 3. Interventions should be tailored to the athlete based on their injury and the demands of their sport. Once an athlete can perform their basic ADLs pain-free, treatment should be individualized to the sport they play. If the sport requires power, incorporate power-based movements. If the sport requires multiplanar motor control, incorporate multiplanar motor control drills. Prepare the athlete for the sport they play. If you’re working with an athlete recovering from an upper extremity injury, use these guidelines as your athlete progresses toward a return to the field.
