HealthForce NH Giving Care Plan Priorities for 2025 Thank you for taking the time to participate in this important survey! After going through the process of creating a status update for the Giving Care Plan, we wanted to find a way to engage with the broader health care community in New Hampshire and those included as a "Responsible Party" in the original Giving Care Plan to select priorities for 2025 that we at HealthForce can help steward. On the next page you will find 20 options from which we are asking you to select your top five, but want to share how we came to that list of 20. After the status update, we removed those marked as "green" (as they're already well underway), those in the "governance" category (as that's much of the work of HealthForce itself), and then included a subset of those in "red" with a timeline of immediate, within three-months, and within six-months, those in "yellow" with a timeline of immediate and within three-months, and any that had originally been marked "high priority." There will be a final optional open-ended question that will allow you to bring forth another priority from the plan that we didn't include if you think it should be in the 2025 list of priorities. If you have questions about this, please feel free to reach out to Senior Director, Kate Luczko, at kate@healthforcenh.org The deadline for this survey is end of day on Wednesday, October 9th. https://lnkd.in/eWvK4-Uq

This initiative engages a collaborative group of non-profit organizations serving residents throughout New Hampshire. Thank you for considering becoming a volunteer driver! Learn more at:
