The increase in medication use and misuse is creating unmanageable health costs for employers and health risks for their plan members. Tria Health's pharmacists work telephonically with members and their physicians to ensure medications are safe, affordable and effective. Tria Health is driven by an understanding that pharmacists make a difference in helping patients manage their chronic conditions. Rising health care costs are a concern for employers and health plans. Chronic conditions have a big impact on these costs because they represent 86% of total health care spend. Medication management is key to effectively managing these conditions, yet 50% of people don’t take medications as prescribed increasing health risks and costs.     🌟 Tria Triumph: GLP-1’s are Not for Everyone! 🌟 A patient with diabetes and a history of a heart attack initially faced GI discomfort from metformin IR. After a change in medication to Ozempic, their GI symptoms worsened dramatically. The patient described the issues as “wicked” with severe gas, nausea, and diarrhea. Our Tria Health pharmacist stepped in, thoroughly investigating the timeline of symptoms and medication changes. By pinpointing Ozempic as the likely cause, the pharmacist recommended discontinuing it and returning to metformin, this time in the extended-release (ER) formulation. Today, the patient’s HgbA1C and blood glucose levels are at goal, thanks to careful medication management and personalized care.     🌟 Tria Triumph: A Lot Can Change in a Year!🌟 We're thrilled to celebrate another patient's incredible journey with Tria Health! Within a year participating in our Choose to Lose (CTL) program, our patient achieved significant health improvements: A1C: Improved from 6.4% to 5.7% BMI: Dropped from 38 to 33 Weight: Reduced from 230 to 203 lbs With guidance from a Tria health coach and the use of weight loss medication, she transformed her lifestyle, gaining a deeper understanding of nutrition and how it impacts her metabolism. Not only did she shed pounds, but she also discontinued regular chiropractor visits, alleviating low back pain and musculoskeletal issues. Perhaps most impressively, after nine months, she successfully transitioned off weight loss medications, maintaining her progress through healthy habits alone. Her stress levels have decreased, thanks to regular exercise and time spent outdoors, allowing her to unplug and recharge.     🌟Tria Triumph: Transforming Health on the Road 🌟 Meet our mid-50s patient, a frequent traveler with a busy schedule, who decided to take control of his health by enrolling in our "Choose to Lose" program. With 80% of his time spent on the road, he faced challenges like frequent dining out and limited physical activity. But with the guidance of our health coach, he made powerful changes. Starting with healthier meal choices and increased water intake, he swapped out burgers for high-protein, nutritious options. Within just a month, he shed 20 pounds and found a renewed passion for fitness. He began with morning walks around his hotels, gradually extending them to 3–4 miles, and even added running into the mix. His dedication paid off—after six months, he lost an incredible 75 pounds, 20% of his body weight, and maintained it for another six months! Not only did he gain a new wardrobe and confidence, but his doctor also confirmed his success with healthy bloodwork results.
