A couple years after a failed spinal fusion done elsewhere, this patient came to Dr. Hardeep Singh with persistent back pain and shooting pain down their right leg. Because new bone had failed to grow around the cage and bone graft that had been put in, the L5/S segment (see diagram) was moving abnormally and irritating the nerve. This added stress to the screws and rods as well, leading to one of the screws eventually breaking. To be minimally invasive, Dr. Singh performed ALIF (Anterior Lumbar Interbody Fusion) surgery with the help of the vascular surgery team. Entering through the stomach and carefully moving the belly contents aside, Dr. Singh pulled out the old cage and put in a new one that fits the disc space better and allows the bone to heal more reliably. After putting the new cage in, Dr. Singh and Dr. Bryan Heard, MD PhD FRCSC used augmented reality technology from Augmedics to make small incisions in the back, remove the old screws, and insert new ones far more efficiently.
