A public foundation improving well-being in Arizona by addressing root causes & broader issues that affect health.
About us
Vitalyst Health Foundation has three goals: 1) Collaborative Networks working to improve the conditions that impact health. 2) Strong Community Partners equipped to advance health equity. 3) Informed Decision-Makers prioritizing community health and well-being. Our goals span across four priorities: Civic Health - Supporting long-term efforts that improve active community participation. Food Systems – Strengthening food systems across rural and urban areas in Arizona. Health Care Integration – Supporting the integration of physical, mental and social determinants of health. Housing – Supporting statewide housing initiatives along the spectrum of housing. Vitalyst Health Foundation has been an Arizona nonprofit corporation since 1907. For most of its existence, the nonprofit operated one or more hospitals in the Phoenix metropolitan area, including St. Luke’s Medical Center and St. Luke’s Behavioral Health Center in Phoenix, Arizona and Tempe St. Luke’s Hospital in Tempe, Arizona. In 1995 all hospital facilities were sold to OrNda HealthCorp and the nonprofit converted to an independent health foundation known initially as St. Luke’s Charitable Health Trust (1996-2000), then as St. Luke’s Health Initiatives (2000-2016) before changing its name in March 2016 to Vitalyst Health Foundation.

Y también felicitaciones a Floribella Redondo-Martinez. We are happy to share more congratulations for 2016 and 2024 Systems Change Grant Recipient, Arizona Community Health Workers Association Inc and Floribella. A true testament to how systems change work is an ongoing effort.
Floribella Redondo-Martinez spent 30 years serving as a promotora de salud (community health worker). Her 20 years working as a migratory farmworker in Yuma, Ariz. gave her a unique lens to speak to the inequities plaguing her peers. She has sought resources to assist with the health disparities impacting farmworkers in Yuma County and beyond and has created a community health worker workforce, Arizona Community Health Workers Association, Inc., to fight for the rights of farmworkers and under-resourced communities. Congratulations Floribella on your RISE award! Read more at https://lnkd.in/ei9qiJn9 #MercyHonors #MercyCares4AZ #BecauseWeCare

Congratulations to Damien Browning of Steps to Recovery Homes, 2022 Systems Change Grant recipient, on his RISE Award from Mercy Care.
Inspiring change within a community can start with anyone. Those who are on the front lines, who speak with the members we serve, who hear their stories, are the ones who make a difference. Damien Browning co-founded Steps to Recovery Homes in 2013. Ten years later, the organization now has its own building, a men’s facility, a women’s facility, a transitional house, a job development program, a resale store, and an outpatient treatment center. Damien has generously shared his journey with others and has inspired others to continue in their recovery. He is a living example that reentry and long-term recovery is possible. Congratulations Damien on your RISE award! Read more at https://lnkd.in/ei9qiJn9 #MercyHonors #MercyCares4AZ #BecauseWeCare
