MASH is the New NASH: What’s in a (Disease) Name? Ever wonder why disease names change? It’s more than just a name swap—it's about clarity, accuracy, and, sometimes, reducing stigma. Take diabetes, for instance. What was once known as "juvenile diabetes" is now "Type 1 diabetes," and "adult-onset diabetes" has become "Type 2 diabetes." This shift helps distinguish between the autoimmune nature of Type 1 and the metabolic nature of Type 2, guiding doctors to more precise diagnoses and treatments. Fast-forward to today, and we see a similar transformation with liver diseases. Non-alcoholic fatty liver disease (NAFLD) has been renamed metabolic dysfunction-associated steatotic liver disease (MASLD), and its more severe form, non-alcoholic steatohepatitis (NASH), is now metabolic dysfunction-associated steatohepatitis (MASH). These names may seem like tongue twisters, but they represent years of effort to better describe the diseases. Previously, the old names told us what the diseases weren’t—they weren’t related to alcohol—but failed to explain what they were. Both MASLD and MASH involve fat accumulation in the liver, but MASH takes it a step further, causing inflammation that can lead to scarring and cirrhosis. This severe scarring is often linked to alcohol abuse in the public mind, but with MASH, it’s a different story. Here's the kicker: MASH is now the leading cause of liver transplants in women in the U.S., and it’s poised to become the top cause overall soon. The new names connect these liver issues to broader metabolic problems like high blood pressure, high blood sugar, and high triglycerides, making it clearer for both doctors and patients. There’s another layer to this change. The term "fatty" in the old names was a stumbling block for many patients, making it harder for them to confront their condition. By dropping this term, the new names aim to be more inclusive and less stigmatizing. Not everyone has embraced the new names, but they’re here to stay. They provide a clearer understanding of diseases, help in diagnosis and treatment, and ease patients' emotional burdens.
